Handy Forms & Information

Absence from School

If your child is unwell and unable to attend school, please call the school on 01420 525010 between 8.15am and 8.55am each day of their illness.  Your child will need to remain off school for 48 hours after their last bout of vomiting or diarrhoea.

We encourage parents and carers to arrange all medical appointments outside of school hours, where possible.  Should your child need to be absent for school for any reason, you will need to complete a Request for Absence from School form.  Click here to complete

Requests to speak to a teacher

Staff at Bentley are always keen to resolve issues where we can and always want to know any concerns. Please ring or email rather than use Seesaw as this platform is for child to teacher communications only. If you need to request a meeting with a member of staff please download the form at the foot of this page and return it to the office.  The teacher will get back to you as soon as is possible.

Please allow the member of staff time to put changes in place. If you are still concerned please look at our pathway of support on our Complaints Policy.

Free School Meals

Eligibility for free school meals depends on the circumstances of the parents or carers.  Full details of the criteria and are available by clicking here.

You can check your eligibility by clicking here.

Administration of Medication

If your child needs to take prescribed medication during the school day, our staff are able to administer this on your behalf, with your consent.  Please click here to complete the necessary paperwork, and deliver the medication to the school office.

Leaving School Independently

Children in our Junior years are able to leave school independently at the end of the school day; if you wish your child to do so, please complete the form at this link.

We regularly remind children who are meeting someone off the school grounds, to return to school if their adult is not there to meet them, and would encourage you to do the same.  Should this occur, the school office will get in touch to let you know your child is still in the school building and you will need to come and collect them.

Passport Applications

As a school we may be able to help with passports applications. Please let the Headteacher or Deputy Headteacher know if you would like to request this service. Please ensure that we have all the paperwork with the child's application as we have to check it very carefully in order to authorise it. You can expect that this will be completed within a week of handing this into school unless we let you know that this will not be possible for us to complete at this time.

There is a cost of £10 per application which must be paid for in advance through the school office. This money is paid into the school budget.
